When nominating someone on the yellow rose awards please remember that a panel will decide on your description of that person, as the panel may have never met them before. so please include as much info into this form as possible, All winning nominations will be notified by the end of August 2008, you will then have the task of keeping your winning nomination a secret and getting your winner to the venue for their award.
